Covered California for Small Business –
New Blue Shield Plans

Starting July 1, Covered California for Small Business (CCSB) is offering new Blue Shield plans, providing more options for enrollees. These plans include the Access+ HMO Network with Platinum, Gold, and Silver metal tier options, as well as the Bronze Trio HMO 7000/70. The two most popular Blue Shield High Deductible Health Plans (HDHP), Silver Full PPO Savings 2300/25% and Bronze Full PPO Savings 7000 plans, are also now available.

All of these plans offer benefits such as Wellvolution, Teladoc Mental Health, Nurse Help 24/7, LifeReferrals 24/7, and the Blue Card program for when members are outside of California.

Allied Benefit Suite (ABS) offers market-leading dental, vision, life, disability insurance, and a personal protection plan designed for groups with 2 to 100 employees. With one invoice, one application, and a single administrator, ABS includes benefits from Delta Dental, VSP, AXA Life, and Personal Protection Plan.

When a group purchases a Delta Dental Small Business Plan, they will have access to any or all of the additional top-tier benefits offered through the Allied Benefit Suite.

Plan summaries, enrolment and change forms, and information about Allied Benefit Suite and our other carriers are always available through Claremont. Program Features

The comprehensive Allied Benefit Suite (ABS) offers high-quality dental, vision, life and long-term disability coverage, and a personal protection plan. Designed for groups with 2 to 100 employees, this convenient benefit solution is a natural extension of Delta Dental’s Small Business Program (SBP).

Through ABS, employers can add cost-effective premier ancillary benefits from Delta Dental, VSP, AXA Life, and Personal Protection Plan, all with consolidated billing and top-tier service.

  • Group size 2 to 100 employees.
  • Single administrator, single invoice.
  • No administrative fees.
  • Eligible employees: full-time, permanent employees only; no part-time or temporary employees are eligible.
  • Eligible dependents: spouse/domestic partner and children up to age 26.
  • Broker commissions are a flat 10% for all lines of coverage.
  • No out-of-state restrictions.

Product Overview

Delta Dental products are the key component of ABS. In order to access the other products in ABS, a group must first sign up for Delta Dental coverage. Once a group has unlocked access to the other products, they can purchase one or as many of the products necessary to fit their employee’s needs.

Delta Dental PPO and DeltaCare USA
Delta Dental has long been Allied’s partner for its Small Business Program (SBP), which offers the flexibility to choose the benefits that best fit your clients’ workforce. Get valuable dental benefits at affordable stable rates. ABS can be customized to the unique needs of your clients.

VSP
VSP is committed to the health and happiness of your small group’s employees. No hassles, no excuses. Just access to high-quality care and easy-to-use benefits with one simple plan design:

– $10/25 copays.
– $150 frame allowance.
– 12/12/12 frequency.
– $130 contact allowance.

  • VSP Choice network.
  • No participation or contribution requirements.
  • Waivers allowed.
  • Ineligible industries match Delta Dental rules.
  • Voluntary plans are allowed.
  • 2-year rate guarantee to match Delta Dental.

Note: The new VSP program no longer requires enrollee and dependent eligibility to mirror the Delta Dental enrollment. With this new enhancement, there are now additional requirements for a new group submission: group application, quote with rates, enrollment forms/census – separate enrollment forms and/or census must be submitted since enrollment is not tied to Delta dental forms.

AXA Life and Disability
Industry-leading life and disability products that provide financial security for policyholders and their families. AXA offers group term life and long-term disability plans to give employees financial protection for themselves and their families in the event of a loss of income.

Features include:

  • Basic life:
    – Flat $15K, $25K, $50K, full guaranteed issue and matching AD&D.
    – Age reduction to 65% at age 65, 50% at age 70.
    – Less than 10 employees: age banded based on the average age of the group.
  • Long-term disability (LTD):
    – Up to 60% income replacement.
    – Maximum benefit up to $10,000 per month. Minimum benefit $100 or 10%
    – Benefit duration: Social Security normal retirement age.
    – Employee Assistance Program (must request): telephonic and three face-to-face counseling sessions.
    – Groups must be in business for at least two years.
    – LTD must be sold with an AXA life plan.
  • No dependent coverage.
  • 100% employee participation required.
  • 100% employer-paid contribution required.
  • Waivers are not allowed.
  • Full-time employees working at least 30 hours per week are eligible.
  • Certain SIC codes are ineligible.
  • No voluntary plans are available at this time.
  • 2-year rate guarantee.

Personal Protection Plan
High-value low-cost services that provide access to 24/7 doctor access, legal services, global emergency travel assistance, and ID theft resolution. Rest assured knowing that this plan will provide peace of mind that extends beyond traditional benefits offerings.

Features include:

  • Single plan that covers all four services.
  • Dependent coverage: entire family, including employee, spouse/domestic partner, and dependents up to age 26 (legal dependent status is based on the laws of the state where the employee/dependent resides).
  • 100% employee participation required.
  • 100% employer-paid contribution required.
  • Waivers are not allowed. All employees must be covered.
  • No ineligible industries.
  • No voluntary plans available.
