Sell a Bundle, Earn a Bundle!

To earn a bonus, sell new specialty lines of coverage with the same employer group and initial effective dates of coverage between August 1 and December 31, 2021. Sell stand-alone specialty lines of coverage, or add specialty lines of coverage to a new or existing Humana group medical customer.

Small Group – 5-99 Eligible Enrolled Employees

$500 bonus when you sell dental (5 enrolled) + vision (2 enrolled) coverage.
$1,000 bonus when you sell dental (5 enrolled) + vision (2 enrolled) + life coverage.

Large Group – 100+ Eligible Enrolled Employees

$1,000 bonus when you sell dental + vision coverage.

Specialty lines of coverage include group dental (including voluntary dental), group vision (including voluntary vision), group term life, and accidental death & dismemberment (AD&D). To learn more, download the Bundling Bonus flyer.

Coverage Features

Dental

Unlimited Annual Maximum plan available for all types of services, including Implants.
Extended Annual Maximum included on $1000, $1500, and $2000 maximums – 30% coinsurance coverage after network discount and maximum benefit is reached. Does not apply to implants or ortho services.
Waive preventive services rider on Annual Maximum (same as DPM on Delta).
No waiting period for 10+ enrolled.
Ortho available for child only and/or adult and child on 2+. Invisalign is covered as an alternative treatment for ortho, subject to policy provisions.
12-month rate lock.

Starting with new business effective January 1, 2020, all PPO, Preventive Plus, and traditional Preferred plans will include these new features in preventative services:

Additional routine cleaning and exam now includes three routine cleanings and exams per calendar year (two previously allowed).
Periodontal cleanings are now covered as a preventive service (100%). Four periodontal cleanings per calendar year now covered under preventive services (previously these cleanings were covered as a basic service, subject to deductible and coinsurance)
Increased age limits on select services. Sealants and topical fluoride treatment are now covered through age 16.
Out-of-network (OON) preventive services are now being reimbursed at the 95th percentile and will result in 93% of OON claims being covered in full. OON urgent care for basic and major services is reimbursed at the 90th percentile.

$100, $130, $150, $160, and $200 frame allowance options.
No employer contribution required.
Three plan platforms (Exam Plus, Full Coverage, Materials Only).
EyeMed Insight nationwide network includes both retail providers and private practice doctors.
– 75,000+ participating providers (52,000 independent/22,000 retail).
2-year rate lock.

Basic Life/AD&D

Conversion/waiver of premium.
Accelerated death benefit.
AD&D riders included: seat belt/airbag/helmet, paralysis, repatriation, common carrier, education benefit, child care benefit, and coma.
Optional dependent coverage.
Three age reduction schedules available (#3 offers the best).
2-year rate lock.

Voluntary Life (Participation – Five Lives Enrolled)

Increments or percentage of salary up to $500,000.
Flat benefits for spouses and children up to age 26.
