Founded in 1955, Delta Dental is the nation's largest and most experienced dental benefits provider. As a not-for-profit health care service plan, Delta Dental offers members a full spectrum of plan designs including HMO, PPO, and managed fee-for service options.
The Delta Program of Strategies for Small Businesses offers two choices:
- Classic Plan Designs for 5-49 employees
- Options Plan Designs for 50-99 employees
Both programs make available 4 plan designs to address employer and employee needs:
- Delta Dental Premier (FFS)
- Delta Dental PPO
- DeltaCare (HMO)
- Delta Dental Voluntary PPO
Delta Dental Premier (FFS)
A managed fee-for-service plan, Delta Dental Premier offers optimal freedom of choice through agreements with over 91% of dentists in the state of California. Members may visit any licensed dentist; out of pocket costs are kept to a minimum by visiting a Delta dentist. Delta Dental Premier can solve employer needs for out-of-state and worldwide coverage.
Delta Dental PPO
Delta Dental PPO offers the advantages of a fee-for-service plan with significant in-network provider savings. Members save money by utilizing over 20,276 Delta PPO locations in California, or can access any non-contracted provider at a higher rate for services provided.
A value-added DHMO plan with zero to no co-payments, and no deductibles or annual maximums, DeltaCare provides up to three cleanings a year, comprehensive orthodontia treatment, and orthodontia takeover from other plans. A full service, affordable option for dental care, the DHMO network provides access to over 4,340 dentist locations in California.
Delta Dental Voluntary PPO
Available to groups of 5-99 employees, the Voluntary Plan is open to all industries. Waiving the $50 annual deductible for diagnostic and preventive services, and offering optional child orthodontic coverage to groups of 25+ enrollees, this plan from Delta presents another way to enhance the group benefits portfolio.
Maximum Coverage Options
Delta Dental also offers the opportunity to put together the value of a dental HMO with the freedom of a managed fee-for-service plan. A dual choice product allows members the opportunity to move from one plan design to another at the group's anniversary.
EyeMed Vision Care
A leader in vision plan administration, EyeMed provides competitively priced coverage, comprehensive plans, and access to over 23,000 providers at over 12,000 California locations.
Providing convenient locations, extended hours and one-hour service through name brand outlets - such as LensCrafters, Site for Sore Eyes, Sterling Optical and other recognizable names - members now manage to add quick, efficient, professional and affordable vision care into their budget and busy day.
